Exit Declaration of Corpses, Skeletons and Coffins
I. In principle, the exit should be made from the port where the body is embalmed. Before the exit of the corpse or skeleton, the consignor or his/her agent shall declare to the customs at the exit port and provide the following materials as required:
(1) The Health Quarantine Declaration Form for the Entry and Exit of Corpses and Skeletons;
(2) The valid identity certificate of the deceased;
(3) The death certificate issued by a medical institution at or above the county level, or the death appraisal certificate issued by the public security or judicial department, or other corresponding notarized materials;
(4) If it is necessary to transport the corpse or skeleton out of the country, the consignor or his/her agent shall obtain the embalming certificate for the entry and exit of the corpse or skeleton, the anti-corrosion certificate, and the health supervision declaration form for the entry and exit of the corpse or skeleton issued by a unit engaged in international corpse transportation services recognized by the competent funeral and interment department of the State Council.
(5) The identity certificate of the consignor or his/her agent.
II. If it is necessary to transport the corpse or skeleton out of the country from a port in a different place, the consignor or his/her agent shall apply to the customs at the place where the body is embalmed for quarantine inspection. If the quarantine inspection is qualified, the customs shall issue the "Health Quarantine Certificate for the Entry/Exit of Corpses/Coffins/Skeletons".
※ When transporting the corpse or skeleton out of the country, the consignor or his/her agent shall declare to the customs at the exit port with the following materials:
(1) The valid identity certificate of the deceased;
(2) The identity certificate of the consignor or his/her agent.
III. If a person dies during the entry, exit or transit, and it is necessary to transport the corpse into the country, the consignor or his/her agent shall declare to the customs and submit the following materials:
(1) The Health Quarantine Declaration Form for the Entry and Exit of Corpses and Skeletons;
(2) The valid identity certificate of the deceased;
(3) The valid death certificate or the death appraisal certificate issued by the public security organ.
IV. For those that fail the health quarantine inspection, relevant quarantine measures shall be taken in accordance with the regulations. In cases where cremation is required according to the regulations, but the consignor or his/her agent disagrees with the cremation, entry or exit shall be prohibited.
